None of the heroes die. Ever.
At the Red Wedding, Catelyn sees Arya gliding mysteriously through the galleys. Ned Stark (not dead after all, what do you mean he’s dead???) stumps through the crowd and slams Cersei’s diary down on the table. In it, she has made the classic woman’s mistake of describing her affair with Jaime in detail. “Sentiment has been her undoing”, Ned announces smugly. The Freys and the Boltons immediately realize what a great guy he is and team up with him to take on the Lannisters.
Arya reveals her time away from her family has turned her into a raging psychopath. Ned and Catelyn make no attempt to help their daughter deal with her trauma. They’re vaguely sad about it, but she’s crazy now and there’s just nothing they can do, so they let her do her own thing and never really trouble about her.
Arya heads to the Wall to find Jon. He left when she was just a child, so obviously she is now in love with him. Jon doesn’t really feel that way about Arya, but is strangely unable to assert himself about it (despite leading the whole of the Night Watch and asserting himself about everything else on a daily basis) so he just goes along passively with their relationship. Eventually Arya throws herself in front of a White Walker to save him, and dies. She lives on as a ghost Jon hallucinates sometimes, and makes out with while she insults him.
